Travel Notices are required for all teams traveling to play in another state for a tournament or for friendly games. This is an official notice to LSA of who is traveling with your group – both your team and guest players.
There are four types of travel notices –

US Youth Soccer Sanctioned Events
Non US Youth Soccer Sanctioned Events
Application for Foreign Travel
- It allows the office to estimate the time needed for travel processing and make adjustments to the workload. Sometimes there are ten (10) and other weeks there may be as many as 70.
- It keeps the office from having to re-approve a travel due to the injury or illness of players who are replaced by a guest player. Yes, if your roster changes it must be re-approved.
- It should make your job easier since you can fill out all your planned travel at once and send them in to be processed. You won’t get caught trying to get them in at the last minute.
We will verify receipt of your travel, when it is received. The travel notice will be emailed to you once it is processed. If you need them to be faxed please state that on the notice. Make sure that all information in legible on the notice so that we can process it quickly.
Why are travel notices necessary? LSA retains a copy of all travel notices as proof that team activities are sanctioned and for insurance records, including liability and medical coverage. Failure to notify LSA of travel may result in sanctions against the coach, the team or club and/or the loss of medical insurance coverage.
USASA Adult Travel Notice (LSA Bylaw 409)
- Obtain the Application to Host – This should be on the host tournament page but if it is not it will be on the State Association Page. This application has the information you will need to fill out the travel notice.
- Obtain an office LSA roster from your registrar.
- Fill out the form
- Submit the three documents to LSA via mail, fax or email. Also include any guest player forms - - be sure to put a line through players who will not be traveling with the team. Please remember that the size of the team cannot exceed the maximum roster size as stated in LSA policy.
- LSA will review your travel, place the approval stamp on it and return it to you the week of your travel.
- Be sure to put the approved travel with your teams forms, you will be asked for proof of travel when checking in at the tournament.
US Youth Soccer Travel (LSA Policy 308)
- Obtain the Application to Host – This should be on the tournament page but if it is not it will be on the State Association Page. This application has the information you will need to fill out the travel notice.
- Obtain an office LSA roster from your registrar.
- Fill out the form
- Please use the same team name as the name on your LSA Roster
- Your Region III State Association is Louisiana Soccer Association
- Fill out the Tournament Information from the Tournament’s Application to Host.
- Submit the three documents to LSA via mail, fax or email. Also include any guest player forms - - be sure to put a line through players who will not be traveling with the team. Please remember that the size of the team cannot exceed the maximum roster size as stated in LSA Policy 310 Section B 2.c.2.
- LSA will review your travel, place the approval stamp on it and return it to you the week of your travel.
- Be sure to put the approved travel with your teams forms, you will be asked for proof of travel when checking in at the tournament.
Exemptions to the Travel Notice Rule are events sponsored by US Youth Soccer – National Championship Series, Presidents Cup, ODP and Sub Regional Premier League. We have asked for an annual Travel Notice for Regional Premier League Teams as a formality in the past.
Non US Youth Soccer Travel (LSA Policy 308)
Coach or Manager
- Coach or Manager must fill out and return the Notification of Non-US Youth Soccer Travel to the LSA office. This form must be received three (3) weeks prior to the first day of travel.
- Complete passes and laminate when the office sends them out.
LSA Office
- Prepare the following documents for the travel
- Travel Notice
- Generic Team Roster
- Generic Team Player Passes
Please note that generic passes and rosters can only be used ONCE - - for the event named on the Notification of Non-US Youth Soccer Travel.
Failure to adhere to the policy set forth by the LSA Executive Committee in regards to Non-US Youth Soccer Travel will result loss of insurance benefits for subsequent events.
Instructions for Applying for Foreign Travel

- Request necessary state forms
- Request U.S. Soccer required forms or print below:
- U.S. Soccer Application for Foreign Travel
- Ted Stevens Amateur Sports Act for Foreign Travel
- Complete all State Forms along with the US Soccer Forms
- Documents Required for USSF Approval:
- U.S. Soccer Application for Foreign Travel, approved by appropriate Affiliated Member in bottom right corner
- Signed Ted Stevens Amateur Sports Act for Foreign Travel
- Copy of Official brochure, pamphlet, invitation or other applicable material of the tournament or games Host.
- Copy of Official roster of traveling team in good standing, which includes all players and team officials, and is signed/stamped by the appropriate Registrar.
- Check for the non-refundable Travel Application Fee of $50 made payable to U.S. Soccer
- Submit completed forms to your State Association office for their review and state certification of U.S. Soccer forms
- State office or applicant submits certified forms directly to U.S. Soccer at least thirty (30) days prior to the date of departure. (Applications not received 30 days prior to departure may be denied by U.S. Soccer.)
